2008 Application for 
HDS Approved Schooling Show Judge

Instructions:

1. Application should be made on or before December 1 of the year prior to the desired listing.
   
   
2. Supporting documents, as indicated, MUST accompany application.  Applications that are not complete will not be considered.
   
   
3. Send application to HDS Schooling Show Chair.  Contact information can be found in HDS newsletter or website.
   
   
4. To be considered for HDS recognized schooling shows, judges must meet below criteria. 
   
   
5. Only approved HDS schooling show judges can be used for schooling shows applying for HDS recognition and year end awards.
   
   
6. Approved schooling show judge applicants will be listed on HDS Website. The list can be mailed on request.

Criteria for HDS Schooling Show Judge

The following criteria MUST be met to be added to the HDS Approved Schooling Show Judge List.  Please check applicable boxes. Submit form with supporting documentation, as indicated.

1	USDF member in year for which listing requested
2*	“L” Program Graduate (or “r”, “R”, S, etc.)  If you believe you have qualifications parallel to or higher than this, submit these for individual consideration.
3*	Scribed in a recognized show (USDF/USAE) minimum 4 hrs for an “R” judge or above within the past year. (See form at http://www.usdf.org/Programs/LProgram/index.asp)
4*	Attended 2 (total approx. 6 hours) HDS approved seminars.  If there are no HDS seminars offered, these requirements may be fulfilled by “outside” seminars (e.g., USDF conference) Attach form  (Explain)
